Products sold through Human Enhancement Clinic are classified as a schedule 4 medication and require a prescription, therefore you are required to fill out this registration form and medical questionnaire. Once completed one of our doctors will order the appropriate blood tests if required and once the test results are available will determine the most appropriate treatment. Our medical staff will support you along the process.
